Maryland licenses clinical alcohol and drug counseling and certifies the tiers below it, all through the Board of Professional Counselors and Therapists.
The ladder runs from Board-authorized trainee status through two state certificates (CSC-AD, then CAC-AD) to two licences (LGADC, then LCADC).
The certificates tie you to a State-certified or licensed agency under supervision; independent practice belongs to the LCADC alone — the LGADC diagnoses too, but under clinical supervision, and a CAC-AD may supervise others with Board approval.
Each licence and certificate passes a national exam plus Maryland's law exam and renews on two-year terms with 40 CEUs.

Alcohol and drug trainee (Board-authorized status), certified supervised counselor–alcohol and drug (CSC-AD), certified associate counselor–alcohol and drug (CAC-AD), licensed graduate alcohol and drug counselor (LGADC), and licensed clinical alcohol and drug counselor (LCADC).
A sixth title, certified professional counselor–alcohol and drug (CPC-AD), survives in § 17-401(b)(1), but COMAR .06 has no initial-qualification rule and the Board may not reinstate a lapsed CPC-AD — the research treats it as legacy.
State licences and state certificates issued by the Maryland Board of Professional Counselors and Therapists under Health Occupations Article, Title 17 and COMAR 10.58.07.
Practice protection attaches to clinical alcohol and drug counseling (§ 17-301(a)); a Board certificate authorizes alcohol and drug counseling (§ 17-401(b)(4)).
MAPCB, a private board, is the IC&RC member — not the Board.
Trainee: an associate's degree in a health and human services counseling field, or 15 semester credit hours including ethics (COMAR .09C).
CSC-AD: an associate's plus a minimum of 24 semester credit hours of alcohol and drug training (.08C).
CAC-AD: a bachelor's plus a minimum of 33 (.07C).
LGADC and LCADC: a master's with a minimum of 60 graduate semester credit hours (doctorate 90) plus a minimum of 39 semester credit hours of alcohol and drug training and 3 graduate credits each in diagnosis/psychopathology and psychotherapy (.04B, .05B).
CSC-AD: a 6-credit internship or a minimum of 1,000 hours as a trainee.
CAC-AD: that plus not less than 1 year with a minimum of 1,000 hours of clinically supervised experience.
LGADC and LCADC: the same internship-or-1,000-hours item (.04B(3), .05B(3)), and the LCADC adds a minimum of 2 years and 2,000 hours after the master's or doctoral degree, of which a minimum of 1,500 hours are direct clinical counseling services including 100 hours of clinical supervision — at least 50 individual face-to-face, at most 50 group.
Trainee status: 6-year maximum.
Every licence and certificate passes a national alcohol and drug counselor examination approved by the Board plus an examination on Health Occupations Article, Title 17 and COMAR 10.58 (.04B(5), .05B(6), .07C(5), .08C(4)); § 17-302(g) adds the law examination on Title 17.
The trainee authorization sets no examination (.09).
The rules read do not name the national examination.
LCADC scope includes clinical evaluation 'including screening, assessment, and diagnosis for mental health disorders' and supervisory responsibilities for all categories (.05A); licensees may diagnose psychological problems, emotional conditions or mental conditions (§ 17-308(b)).
The LGADC scope lists the same diagnostic items, but every activity is performed under clinical supervision, and it may not practice independently or supervise (.04A, .04F); a CAC-AD may provide supervision with Board approval (.07B(2)).
CSC-ADs and CAC-ADs work in State-certified or licensed agencies and participate in diagnostic impressions for SUDs (.08A, .07A).
§ 17-301(c): 'This subtitle may not be construed to limit the scope of practice of any individual who is duly licensed under this article.'
COMAR .09I(3): an individual licensed as a clinical mental healthcare provider may not apply as a trainee.
The Board is not an IC&RC member.
MAPCB (Maryland Addiction & Behavioral-Health Professionals Certification Board), a private board, is the IC&RC member — it lists the CCDC at the ADC reciprocal level and no AADC credential, so an IC&RC transfer produces a MAPCB certification, not the state licence.
Out-of-state LCADC applicants use COMAR 10.58.07.10, which keeps the national and Maryland law exams subject to a Board waiver.
A licence or certificate may not be renewed for a term longer than 2 years (§ 17-504(a)(2)).
Each renewal period requires a minimum of 40 CEUs, at least 30 in Category A (COMAR 10.58.05.03A); a renewal period shorter than 2 years takes a minimum of 20 Category A CEUs.
An authorization, not a credential: the Board issues a letter authorizing the holder to practice alcohol and drug counseling as a trainee for 2 years, renewable in 2-year increments up to a total of 6 years (COMAR 10.58.07.09J and .09K).
A state certificate authorizing alcohol and drug counseling as an employee of a State-certified or licensed agency, under close supervision of an approved supervisor; it may not supervise or practice independently (COMAR 10.58.07.08B).
A state certificate for alcohol and drug counseling as an employee of a State-certified or licensed agency under moderate supervision; it may not practice independently and may provide supervision only with Board approval (COMAR 10.58.07.07B).
A licence to practice graduate alcohol and drug counseling under clinical supervision while accumulating the supervised experience for the LCADC; the rule says a licensee in this category may not practice independently or provide supervision (COMAR 10.58.07.04A, .04F).
The top licence: clinical alcohol and drug counseling is practice-protected, and the LCADC scope includes diagnosis for mental health disorders and supervisory responsibilities for all other categories (Health Occ. § 17-301(a); COMAR 10.58.07.05A).
A Board approval rather than a separate credential: an LCADC, a health-care provider licensed under the Health Occupations Article with 3 years of documented alcohol and drug counseling experience, a CPC-AD, a CAC-AD with 3 years of documented experience, a licensed clinical professional counselor, a licensed clinical marriage and family therapist, or a licensed clinical professional art therapist (COMAR 10.58.07.02B(3)).
Maryland splits the field into a certificate track and a clinical licence track, both issued by the Board of Professional Counselors and Therapists.
The certificates — CSC-AD and CAC-AD — authorize alcohol and drug counseling: § 17-401(b)(4) says a certificate issued by the Board authorizes the holder to practice alcohol and drug counseling while it is effective.
Practice protection attaches to the clinical side: outside the § 17-301(b) exemptions for students and Board-approved trainees, clinical alcohol and drug counseling requires a licence.
The LGADC is the supervised licence rung; the LCADC is the independent one.
The certified professional counselor–alcohol and drug (CPC-AD) title still appears in § 17-401(b)(1), but COMAR .06 now contains only scope and renewal rules and says the Board may not reinstate a lapsed CPC-AD, so the research treats it as legacy.
The same letters mean different tiers in different states; see CADC vs LADC vs LCDC vs CAC.
“Except as otherwise provided in subsection (b) of this section, an individual may not practice, attempt to practice, or offer to practice clinical alcohol and drug counseling, clinical marriage and family therapy, clinical professional art therapy, or clinical professional counseling in the State unless licensed by the Board.”
The trainee authorization starts at an associate's degree in a health and human services counseling field, or 15 semester credit hours from the alcohol and drug topic areas including ethics (.09C).
The CSC-AD adds an associate's in the field plus a minimum of 24 semester credit hours (37 quarter) of alcohol and drug training covering pharmacology, addictions treatment delivery and ethics, plus any three electives (.08C).
The CAC-AD raises both: a bachelor's in the field plus a minimum of 33 semester credit hours (50 quarter) (.07C).
The licences sit at a master's with a minimum of 60 graduate semester credit hours (doctorate 90), plus a minimum of 39 semester credit hours of alcohol and drug training and 3 graduate credits each in diagnosis and psychopathology and in psychotherapy and treatment of mental and emotional disorders (.04B, .05B).
A master's outside the field can qualify through a program of studies the Board determines substantially equivalent in subject matter, a minimum of 60 credit hours in counseling including the required alcohol and drug coursework (.04B(1)(c), .05B(1)(c)).
For which entry tiers take no degree at all, see can you be an addiction counselor without a degree?
Every licence and certificate shares one prerequisite item: an internship in alcohol and drug counseling totaling 6 semester credit hours, or a minimum of 1,000 hours of counseling work completed under an approved supervisor.
The acceptable statuses step up the ladder — as a trainee for the CSC-AD (.08C); as a CSC-AD or trainee for the CAC-AD (.07C); as a CAC-AD, CSC-AD or trainee for the LGADC (.04B(3)); and the LCADC also counts hours worked as an LGADC (.05B(3)).
The CAC-AD then adds not less than 1 year with a minimum of 1,000 hours of clinically supervised experience (.07C).
The LCADC then adds a minimum of 2 years and 2,000 hours of supervised clinical experience obtained after the awarding of the master's or doctoral degree, as a supervisee under an approved supervisor — the statute states the same 2-year, 2,000-hour floor at § 17-302(e).
The LGADC licence expires after 2 years unless extended, up to 6, so plan the hours against that clock (.04G).
Trainee status itself runs a maximum of 6 years (.09J).
For the whole path from coursework to credential, see how to become a substance abuse counselor.
“Completed a minimum of 2 years and 2,000 hours of supervised clinical experience in alcohol and drug counseling: (a) Obtained after the awarding of a master’s or doctoral degree; (b) As a supervisee, as defined under Regulation .02B of this chapter, under an approved alcohol and drug counselor supervisor; and (c) Of which a minimum of 1,500 hours shall be direct clinical counseling services, including 100 hours of clinical supervision, of which: (i) A minimum of 50 hours shall be individual face-to-face clinical supervision; and (ii) A maximum of 50 hours may be face-to-face group clinical supervision;”
Maryland tests twice for every licence and certificate: a national alcohol and drug counselor examination approved by the Board, and a Maryland-specific examination on Health Occupations Article, Title 17 and COMAR 10.58 — the statute calls the second one the law examination on Title 17.
The trainee authorization sets no examination in the rules read.
The rule text read also does not name which national examination the Board approves, so confirm the current exam with the Board before scheduling.
The statute requires both exams before supervised graduate practice: an individual may practice graduate alcohol and drug counseling under supervision only after passing the national examination and the law examination (§ 17-309(c)(2)).
An applicant who fails a licence or certificate exam may retake it within the testing authority's window, or on a Board waiver for exceptional circumstances; otherwise a new application and fee apply (.04D, .07E, .08E).
“Except as otherwise provided in this title, the applicant shall pass: (1) An examination approved by the Board; and (2) The law examination on this title administered by the Board.”
All supervised experience runs through a Board-approved alcohol and drug counselor supervisor, and the approval list is broad: an LCADC; a licensed health-care provider with 3 years of documented alcohol and drug counseling experience; a CPC-AD; a CAC-AD with 3 years of documented experience; a licensed clinical professional counselor; a licensed clinical marriage and family therapist; or a licensed clinical professional art therapist (.02B(3)).
The definition of supervision sets the floor: counseling-skill improvement for a minimum of 1 documented hour in each 40-hour work week, or not less than 2 documented hours per month on a shorter week (.02B(31)).
Intensity steps with the rung — close supervision for the CSC-AD (.08B) and for associate-degree and high-school trainees, moderate for the CAC-AD (.07B) and bachelor's trainees, clinical supervision for the LGADC and master's-degree trainees.
The certificates and trainees must also be employees of an agency or facility certified or licensed by the State, so a private-practice placement will not qualify them.
“Improving counseling skills for a minimum of: (i) 1 documented hour for each 40-hour work week; or (ii) Not less than 2 documented hours per month for less than a 40-hour work week.”
The LCADC scope opens with clinical evaluation 'including screening, assessment, and diagnosis for mental health disorders' and closes with supervisory responsibilities for all categories of licensed and certified alcohol and drug counselors or trainees (.05A); the statute generalizes it at § 17-308(b).
The LGADC's scope list is nearly the same — it too covers diagnosis for mental health disorders — but .04A requires every activity under the clinical supervision of an approved supervisor, and the rule flatly says a licensee in this category may not practice independently or provide supervision (.04F).
The CAC-AD performs biopsychosocial evaluation including participating in the development of diagnostic impressions for substance use disorders, and may provide supervision with Board approval, but not practice independently (.07A, .07B); the CSC-AD participates in diagnostic impressions only as confirmed and signed by an approved supervisor (.08A).
For the certificate tiers, the diagnostic work is contributing to someone else's diagnosis, not making it.

“An individual licensed under this subtitle may provide services involving the application of counseling principles and methods in the diagnosis, prevention, treatment, and amelioration of psychological problems, emotional conditions, or mental conditions of individuals or groups.”
The Board of Professional Counselors and Therapists is not an IC&RC member board; IC&RC reciprocity delivers no Maryland credential.
COMAR 10.58.07.10 sets the LCADC endorsement route: an applicant licensed as a clinical alcohol and drug counselor or its equivalent elsewhere must still pass the Board-approved national examination and the Title 17/COMAR 10.58 examination, subject to the waiver quoted below.
For a master's under 60 graduate credit hours, .10E(1)(b) substitutes not less than 3 years practicing as a clinical alcohol and drug counselor with a minimum of 2,000 clinical hours.
COMAR 10.58.07.11 does the same for out-of-state CAC-AD applicants: parallel exams and waiver (.11B), a course waiver that cannot touch ethics (.11D), and equivalent experience of not less than 2 years and 2,000 hours (.11E); COMAR .12 covers the CSC-AD.
Beside the Board sits the Maryland Addiction & Behavioral-Health Professionals Certification Board (MAPCB), a private body and the state's IC&RC member; it certifies the CCDC at the ADC reciprocal level and no AADC, so an AADC holder reciprocates at the ADC level — a private credential, not the state licence.
For what IC&RC reciprocity transfers, and why NAADAC credentials do not, see IC&RC vs NAADAC.
“The Board may waive any examination requirements for an applicant who satisfies the Board that the applicant: (1) Obtained licensure in a state or country that grants a similar endorsement to license holders of this State; (2) Became licensed in another state or country after passing an examination approved by the Board; and (3) Meets the qualifications otherwise required by Health Occupations Article, Title 17, Annotated Code of Maryland, and this chapter.”
The renewal term cap covers licences and certificates alike: § 17-504(a)(2) says a licence or certificate may not be renewed for a term longer than 2 years.
Renewal takes the fee, the application and satisfactory evidence of compliance with the Board's continuing education requirements (§ 17-504(d)).
The CE rule sets a minimum of 40 CEUs in each renewal period, of which at least 30 must be Category A activities and no more than 10 Category B — or all 40 may be Category A (COMAR 10.58.05.03A); a renewal period shorter than 2 years takes a minimum of 20 Category A CEUs (.03C).
Renewal works by attestation, but the holder must keep the supporting documentation for 4 years for possible Board inspection (.03D).
“A license or certificate may not be renewed for a term longer than 2 years.”
This page summarizes the Maryland statute, rules, board pages and other documents cited above, as read for this state-by-state substance abuse counselor requirements series, current as of September 2026.
It does not list application forms, fees beyond those cited above, or processing times, and it is not a substitute for the instructions the Maryland Board of Professional Counselors and Therapists publishes.
Nor does it cover peer specialist certification in Maryland.
Hour totals, exams and supervision rules change by statute and by board rule; for why the same letters mean different tiers from state to state, see CADC vs LADC vs LCDC vs CAC.
Confirm every requirement with the Maryland Board of Professional Counselors and Therapists before you count hours, register for an exam or apply — and before you take one of the substance abuse counselor jobs in Maryland that asks for it.
For clinical alcohol and drug counseling, yes: Health Occ. § 17-301(a) bars any individual from practicing, attempting to practice, or offering to practice it in Maryland unless licensed by the Board, subject to exemptions for students and Board-approved trainees.
The CSC-AD and CAC-AD certificates authorize alcohol and drug counseling under supervision in a State-certified or licensed agency.
The Board of Professional Counselors and Therapists issues everything; MAPCB's certification is a separate private credential.
Degree and supervision.
The CSC-AD takes an associate's degree plus a minimum of 24 semester credit hours of alcohol and drug training and works under close supervision; the CAC-AD takes a bachelor's plus a minimum of 33 credit hours, works under moderate supervision, and may supervise others with Board approval.
The LGADC is a licence for master's graduates who have passed both exams and are collecting the supervised experience for the LCADC, starting from a 6-credit internship or 1,000 hours of counseling work (.04B(3)).
Only the two certificates require employment by a State-certified or licensed agency; the LGADC rule sets no employment-setting condition.
A minimum of 2 years and 2,000 hours of supervised clinical experience in alcohol and drug counseling, obtained after the awarding of your master's or doctoral degree and as a supervisee under an approved supervisor.
At least 1,500 of those hours must be direct clinical counseling services, and the total includes 100 hours of clinical supervision — a minimum of 50 individual face-to-face hours and a maximum of 50 group face-to-face hours.
The statute sets the same 2-year, 2,000-hour floor at § 17-302(e); the 1,500-hour direct-service minimum and the 100-hour supervision split come from COMAR .05B(5).
Before either, the LCADC also needs the 6-credit internship or 1,000-hour item.
Not at the state licence.
The Board of Professional Counselors and Therapists is not an IC&RC member board; the private MAPCB is, and an IC&RC reciprocity transfer lands there — as a CCDC certification at the ADC level, since MAPCB offers no AADC reciprocal credential.
To get the state LCADC as an out-of-state licensee you apply under COMAR 10.58.07.10, which requires the national exam and Maryland's law exam again, with a Board-discretion waiver for applicants from states granting a similar endorsement; COMAR .11 and .12 set the parallel routes for the CAC-AD and CSC-AD.
The LCADC's scope includes screening, assessment, and diagnosis for mental health disorders, and § 17-308(b) lets licensees apply counseling principles to the diagnosis of psychological problems, emotional conditions or mental conditions.
The LGADC's scope list is nearly the same, but every activity happens under clinical supervision, and it may not practice independently or supervise.
The CAC-AD may supervise with Board approval; the certificate tiers participate in developing diagnostic impressions for substance use disorders — they contribute to a supervisor-confirmed diagnosis rather than making one.
