What to Look for When Hiring a Therapist
Decide which licenses you will accept before you write the posting.
"Therapist" is a job title, not a license.
The clinicians you are hiring hold one of three license families: a clinical social work license (LCSW in many states), a professional counselor license whose letters vary by state (LPC, LMHC, LPCC, LCPC), or a marriage and family therapist license (LMFT).
For most outpatient psychotherapy roles all three can do the work, but payer rules, your supervision structure and the populations you serve may favour one.
See LCSW vs. LPC vs. LMFT for how they differ, and the counselor title map for your state's letters.
Then decide whether you will hire associates.
A pre-licensure clinician (LPC Associate, LMSW, AMFT and similar) can often see clients, but only under a qualified supervisor, and the hours they log count toward their own license.
Hiring associates widens your pool and lowers the wage bill, but it commits a licensed clinician's time to supervision and limits which payers will reimburse the work.
Associate vs. independently licensed sets out what a supervised hour is.
Beyond the license, screen for the populations and modalities the caseload actually needs, comfort with your documentation system and productivity model, and whether the candidate needs a license in more than one state for telehealth.

Post a Therapist Job βCredential Verification Checklist
Look the license up at the state board, every time.
The Joint Commission treats primary source verification as the organization's responsibility, not the clinician's, and a copy of a license card is not verification.
Use the issuing board's online lookup; our counselor licensure by state pages link each board.
Record the date, who checked, what was checked and the result.
- Tier and status: confirm the candidate holds the independent license the role requires, or the associate license and a supervision plan if you are hiring pre-licensure, and that it is active with no restrictions.
- Compact privileges: a counselor practising in your state on a Counseling Compact privilege is verified through CompactConnect, and the privilege does not update automatically when the home license renews.
- Other states: for telehealth across state lines, verify a license in each state where clients are located.
- Exclusion screening: if the work is paid by Medicare or Medicaid, search the OIG exclusion list before hire. See hiring compliance: license verification and exclusion checks.
What to Pay: Therapist Salary Benchmarks
There is no Bureau of Labor Statistics occupation called "therapist", so budget from the three that licensed therapists fall into, and name the one you use.
National medians, May 2025: $59,350 for 21-1018 substance abuse, behavioral disorder and mental health counselors (a combined series that includes counselors without a clinical license), $60,280 for 21-1023 mental health and substance abuse social workers, and $66,940 for 21-1013 marriage and family therapists.
Setting moves these numbers, and not in one direction.
For mental health and substance abuse social workers, the May 2025 median was $72,940 in offices of mental health practitioners but $52,200 in outpatient mental health and substance abuse centers.
For marriage and family therapists, outpatient centers ($74,070) sat above offices of mental health practitioners ($65,810).
Compare against your own setting, not the all-industry figure.
Our therapist salary guide breaks the figures down by state.
Pay structure has legal limits.
Under federal overtime rules, therapists, social workers and psychologists must be paid on a salary basis to be exempt: at least $684 a week under the rules DOL restored in May 2026.
A W-2 therapist paid purely per session or per hour is at risk of being non-exempt, and in January 2026 DOL said an LCSW moved to hourly pay would likely lose the exemption.
A salary guarantee plus per-session incentives is the structure the rules allow.
Writing the Job Description
Name the licenses you accept in your state's own titles.
"Licensed therapist" tells a candidate nothing; "LCSW, LPC or LMFT, or an associate license with supervision provided" tells them whether to apply.
If you accept associates, say who supervises, whether supervision is paid for, and whether their hours count toward licensure in your setting.
State the pay model plainly.
Salary, salary plus per-session bonus, or fee-for-service; the productivity expectation; and how no-shows are treated.
Candidates compare these more than the headline number.
Be exact about the caseload and settings: ages, diagnoses, modalities, telehealth or in-person, and any on-call or crisis duties.
In California, associates and trainees may only work as employees or volunteers, never as independent contractors, so a 1099 posting that invites associates is wrong there.
Our therapist job description template has the full structure.
Onboarding Your New Therapist
Start payer credentialing before the start date.
A new therapist who cannot bill is a salary with no revenue.
The sequence is an NPI, then Medicare enrolment where it applies, then a CAQH ProView profile and the commercial panels.
Since January 1, 2024, Medicare pays LMFTs and licensed mental health counselors directly, but enrolment is required in each state where services are given.
No source publishes how long commercial credentialing takes for a behavioral clinician, so plan for a gap and ask each panel.
Associates bill differently.
Medicare lets a physician, clinical psychologist, NP, CNS, CNM or PA bill an associate's work incident-to their own services under general supervision, but an LCSW-, LPC- or LMFT-owned practice cannot bill Medicare that way.
Commercial and Medicaid rules depend on the plan and the state; Colorado's insurance regulator found 11 of 18 surveyed carriers paying for supervised pre-licensed work.
In the first month, confirm the supervision schedule in writing for associates, walk through your documentation and productivity expectations, and ramp the caseload rather than filling it on day one.
