What to Look for When Hiring a Substance Abuse Counselor
Decide which credential and tier you are hiring before you write the posting.
"Substance abuse counselor" is a job title, not one credential.
In some states the person holds a state licence (Texas LCDC is the state's only SUD-counsellor licence), in others a state-issued certificate (New York's CASAC, issued by OASAS), and in the rest only a private certification exists (California counselors register or certify with one of three DHCS-approved private organizations: CAADE, CADTP or CCAPP).
Our substance abuse counselor career guide maps the whole system, and the CADC vs LADC vs LCDC vs CAC title map shows what the letters mean where you hire.
The licence/certificate split does not track legal force.
The research behind this site's 51-jurisdiction credential pages classifies 33 states as issuing a state SUD-counselor licence at one or more tiers (counting a state as a licence state when any counselor tier is a licence), 6 jurisdictions (five states and DC) as issuing a state certificate, and 12 states as having private certification only — as of the September 2026 research pass.
What matters for you is the rule underneath: some private-certification systems are effectively mandatory in funded or licensed programs (Michigan requires MCBAP certification by funding policy; Oregon requires MHACBO by program rule), while a state licence can be title protection only (Delaware).
Check your state's program-licensing and funding rules, not just the credential.
Then pick the tier the caseload needs.
Tiers run from high-school-floor entry credentials to master's-level independent licences, and hour requirements often run inversely to degree — in Louisiana, the registered addiction counselor (no degree) needs 6,000 supervised-work hours, the certified (bachelor's) tier 4,000, and the licensed (master's) tier 2,000, and only the LAC may practise independently or render a diagnostic impression.
The same letters sit at different floors in different states: "LAC" is Colorado's top licence but Montana's associate-degree licence, and Wyoming's "LAT" is a master's Licensed Addictions Therapist while Arizona's is an entry-level Licensed Addiction Technician.
If the role carries independent caseloads or diagnosis, name the tier your state's law trusts with that — what a CADC can diagnose or bill breaks the scope rules down.
Treat national certification as a signal, not a qualification.
IC&RC does not certify individuals — its member boards do — so the correct check is "certified by [named board], an IC&RC member board."
The IC&RC ADC floor is a high-school diploma, 300 education hours, and supervised experience that steps down from 6,000 hours (high-school entry) to 2,000 (master's) with 300 to 100 supervision hours, but those are the consortium minimums: each member board and each state sets its own figures, and they are often different.
NAADAC's national certifications (NCAC I, NCAC II, MAC) are voluntary on top of a current state credential — they are never themselves a route into practice.
Our IC&RC vs NAADAC guide explains the two systems.
Beyond the credential, screen for the setting the role actually is (outpatient, residential, withdrawal management), comfort with your documentation and productivity model, and any program-specific requirement in your state.
Also check the current map rather than an old posting: West Virginia's first SUD-counselor licensure law took effect June 12, 2026, with the Bureau for Behavioral Health needing to administer it by July 1, 2027 and existing privately certified counselors grandfathered — the credential landscape is still moving.
For roles built on lived experience rather than a credential, see working without a degree.

Verify with the issuing body, every time — and in this field the issuing body is often not the state.
It may be a state licence board (Texas LCDC, under Occupations Code ch. 504 with rules at 26 TAC ch. 562), a state agency (New York's OASAS for the CASAC), or a private nonprofit board that is an IC&RC member.
Look the credential up in the issuer's own directory or lookup, not from a copy the candidate provides.
Record the credential, the tier, the status, the expiration date, who checked, and the date.
- Tier against duties: confirm the tier can lawfully do what the role does. In Louisiana, certified and registered addiction counselors may not render a diagnostic impression; a New York CASAC cannot open a private practice (that requires an Education Department licence); Texas bars the practice of chemical dependency counseling without the LCDC, subject to exemptions. These are state examples — check your own state's scope lines.
- Portability: IC&RC reciprocity runs only between member boards, only for the same credential at the reciprocal level, and the receiving board can add requirements — a counselor moving from a licence state receives a licence, from a certification state a certification. There is no NAADAC-to-IC&RC credential reciprocity; what flows one way is exam scores (an IC&RC ADC/AADC pass counts toward NAADAC's NCAC or MAC).
- Exclusion screening: if the position works in federally paid programs, search the OIG exclusion list before hire. See hiring compliance: license verification and exclusion checks.
- Records rule: SUD patient records follow a separate federal confidentiality rule that is stricter than HIPAA in important ways — read our 42 CFR Part 2 explainer before the first hire writes a note.
What to Pay: Substance Abuse Counselor Salary Benchmarks
There is no Bureau of Labor Statistics occupation for substance abuse counselors alone.
BLS combines them with behavioral disorder and mental health counselors in SOC 21-1018 — a combined series, so name it when you budget.
National, all industries, BLS OEWS May 2025: 25th percentile $47,100, median $59,350, 75th percentile $76,530, across roughly 491,930 workers in the combined occupations.
The combination cuts both ways: the median includes master's-level, independently licensed mental health counselors, so it can run above what a certificate-tier hire commands, while an experienced licensed-tier counselor may sit above it.
Plan pay structures around the exemption rules.
Many SUD counselors are hired at certificate, registered or bachelor's tiers that generally fail the learned-professional duties test for the FLSA overtime exemption, so treat those hires as non-exempt from the start.
A master's-level counselor you hire for independent clinical work falls under the same salary-basis rule as other therapists: to be exempt they must be paid on a salary basis of at least $684 a week under the rules DOL restored in May 2026.
State the overtime treatment in the posting either way.
Writing the Job Description
Name the exact credential and tier in your state's own terms.
"CADC certified" means a state certificate in Connecticut, a private certification in Illinois, and something else again in Oregon — write the issuing body instead: "LCDC (Texas license)", "CASAC or CASAC Level 2 (OASAS credential)", "CCAPP-certified (California)".
If you accept trainee or intern tiers — a Texas Counselor Intern, a New York CASAC-Trainee — say so, and say whether you provide and pay for supervision toward the next tier.
Match the listed duties to a tier your state's law trusts with them.
If the role includes diagnosis or autonomous caseloads, name the tier that can lawfully carry that work, and say what the entry tier does instead.
If your program places clients by level of care, name the framework — payers reference it too (Utah's Medicaid rules, for example, require SUD residential services to follow each patient's ASAM assessment and treatment plan; the ASAM Criteria is published by the American Society of Addiction Medicine, now in its fourth edition).
Our ASAM levels of care guide is a plain-English orientation.
Be plain about pay and hours.
Hourly or salary, the productivity expectation, how no-shows are treated, and — because many certificate-tier hires are non-exempt — how overtime is handled.
Candidates in this field compare supervision quality and caseload mix as much as the headline number, so say who supervises, how often, and whether group supervision counts.
Onboarding Your New Substance Abuse Counselor
Start payer enrollment before the start date.
For a master's-level hire who will bill Medicare, the sequence is an NPI (the mental-health-counselor taxonomy is 101YM0800X), then Medicare enrollment through PECOS or the paper CMS-855I, then a CAQH ProView profile and the commercial panels.
A separate CMS-855I is required in each state where services are rendered.
CMS instructs its contractors to process 95 percent of PECOS applications without development within 15 calendar days — and, when a site visit, development or fingerprinting is needed, 95 percent within 50 days and all within 85 — but those are contractor performance standards, not a promise to any applicant.
No source publishes how long commercial credentialing takes for a behavioral clinician, so plan for a gap.
One cushion exists in the federal rules: a non-physician practitioner can retroactively bill for up to 30 days before the billing-privileges effective date where circumstances precluded enrollment in advance (42 CFR 424.521(a)(1)(i)) — MHCs enroll on the CMS-855I under a non-physician practitioner specialty, so the rule appears to apply to them (the regulation does not name MHCs).
Medicare is a master's-level gate for this role.
Since January 1, 2024, mental health counselors enroll directly under 42 CFR 410.54(a): a master's or doctorate qualifying under state law, at least 2 years or 3,000 hours of post-master's supervised clinical experience, and licensure or certification by the state as "a mental health counselor, clinical professional counselor, professional counselor" — the three titles the rule names.
CMS guidance reads the title requirement more broadly: per the CY2024 physician-fee-schedule preamble and CMS's enrollment FAQs, addiction counselors who meet all of the other requirements may enroll — but only as MHCs — and CMS said explicitly that bachelor's-level certified counselors "would not meet the education requirements."
A high-school- or bachelor's-tier counselor cannot enroll in Medicare at all.
Entry-tier staff reach Medicare only incident-to, and only through a limited list of billers.
Behavioral health services may be furnished under general supervision (since January 1, 2023) as auxiliary personnel, but only a physician, clinical psychologist, nurse practitioner, clinical nurse specialist, physician assistant or nurse-midwife can bill incident-to — a counselor-led practice without one of those practitioners cannot bill an entry-tier counselor's services to Medicare that way.
Auxiliary personnel must meet state licensure law, and where state law is silent the supervision default is direct.
Medicaid and commercial rules differ by state and plan.
Medicaid is a state-plan question: federal rules let states cover supervised non-licensed practitioners under the other-licensed-practitioner benefit where the state's scope act allows supervision and the licensed practitioner bills — Utah's Medicaid manual, for example, lists its SUD counselor tiers among qualified targeted-case-management providers.
Commercially, counselors below full licensure are not credentialed as their own panel members; in Colorado, a state insurance division survey for plan year 2023 found 11 of 18 carriers allowing supervised pre-licensure billing (a definition that includes substance use providers) — one state's result, not a national rate.
Some states set clean-application deadlines (Arizona requires insurers to conclude credentialing within 60 days of a complete application).
In the first month, put the supervision schedule in writing for trainee- and intern-tier hires (in Texas, an intern's hours count only after counselor-intern registration, and the supervisor must be on site until 2,000 supervised hours are logged), walk through documentation and your records-confidentiality rules, and ramp the caseload rather than filling it on day one.
