Bureau of Labor Statistics, Occupational Employment and Wage Statistics, May 2025 release.
Figures are national annual wages for the SOC occupations behind the behavioral health case manager title β there is no BLS occupation of that name; the closest series is a proxy that does not list the title.
Each card below is a separate BLS occupation, surveyed and published independently for May 2025.
One job title maps onto at least three tables β and the broadest of them, the site's proxy for the role, does not list the title among its reported job names:
Social and human service assistants β the proxy series
SOC 21-1093 Β· median $45,930 Β· $22.08/hr Β· 10th pct $34,200, 90th $63,290 Β· 437,860 jobs β the site's proxy for this role; "case manager" is not among its O*NET sample titles
$38,150β$54,350
Mental health & substance abuse social workers
SOC 21-1023 Β· median $60,280 Β· $28.98/hr Β· 132,810 jobs β where BSW/MSW case managers in community mental health and SUD programs count
$47,070β$79,730
Healthcare social workers
SOC 21-1022 Β· median $67,880 Β· $32.63/hr Β· 187,630 jobs β hospital and medical-setting case management; the top of the span
$56,710β$82,240
For contrast β all U.S. occupations
SOC 00-0000 Β· median $50,980 β the proxy median sits $5,050 below it; both social-work medians sit above
$37,590β$80,520
There is no such thing as a case manager salary in the federal wage data β there are three.
The Bureau of Labor Statistics publishes no occupation of that name, and the title maps onto separately surveyed occupations whose May 2025 medians run from $45,930 to $67,880, a spread of $21,950.
Any single number you have seen attached to this job title is a choice about which of those tables to borrow, and the choice is rarely disclosed.
This page shows all three instead of picking one.
The series this site uses as the role's proxy β and the broadest table in the field β is SOC 21-1093, Social and Human Service Assistants: 437,860 jobs, a median of $45,930, the middle half between $38,150 and $54,350, $22.08 an hour at the median.
The occupation is enormous and diffuse by design.
O*NET defines it as assisting other social and human service providers in a wide variety of fields, such as psychology, rehabilitation, or social work, and its respondents report a bachelor's-degree requirement for only 39 percent of jobs.
It is the least specific table a behavioral health title can borrow.
The proxy label matters more here than anywhere else on this site, because the title does not merely fail to match the occupation β it is absent from it.
"Case manager" is not among 21-1093's reported sample titles (Addictions Counselor Assistant, Social Services Assistant, Social Work Assistant, and similar), and O*NET's own keyword search for the term ranks the series eleventh β behind both social-work occupations in the cards above.
The same series is a stronger proxy for peer support specialists, whose "Peer Specialist" title does appear on the occupation's list.
A figure quoted as "the case manager median" from this table is not an estimate of this job; it is the wage record of an adjacent one, borrowed because it is the least wrong.
The two tables above it are the degreed version of the work.
Case managers who hold a BSW or MSW usually count as social workers: those in community mental health and substance use programs land in mental health and substance abuse social workers (SOC 21-1023), a median of $60,280 at $28.98 an hour, while hospital and medical-setting case management counts as healthcare social workers (SOC 21-1022) at $67,880, $32.63 an hour.
Which of the two a job belongs in is decided by the setting, not the title β the same split documented on the LCSW salary page, where the licence is identical and the building makes the difference.
A posting that reads "case manager, BSW required" is describing a job in one of these two tables.
The sharpest line in the release runs between the proxy and the degreed tables.
The 25th percentile is $47,070 for mental health and substance abuse social workers and $56,710 for healthcare social workers β both above the $45,930 median of the proxy occupation.
Position at the bottom quarter of either degreed table already pays more than the middle of the table the unlicensed role borrows.
The rule has one exception at the very bottom: the 10th percentile of the mental health and substance abuse table is $39,740, below the proxy median, so that table's floor overlaps the borrowed distribution rather than clearing it.
From the 25th percentile upward, though, every published figure in both degreed tables sits above the proxy median.
OEWS cannot say why β it measures wages and employment, not causes β and it is the numeric form of the field's standard advice: the degree-and-licence step, not years spent coordinating, is what moves you into a different table.
Why the unlicensed role earns less is structure, not skill.
Funded case management bills as care coordination under Medicaid's targeted case-management rules, where qualifications are set by each state's program policy rather than by a licence, and where the service is defined as arranging and monitoring services rather than delivering them β what that means day to day is on the behavioral health case manager career guide.
The higher-paying work on the same teams is clinical, and clinical billing is gated by payer credentialing that requires independent licensure β the same mechanism behind every associate pay gap, explained in can an associate bill insurance?
What no table in the release can do is isolate the title.
There is no case-manager split inside 21-1093, no setting axis for any of the three occupations, and no credential field; the percentiles are positions in each occupation's whole distribution, not an experience ladder, so the 25th percentile is not an entry-level salary.
And there is no named, dated pay survey of behavioral health case managers to fall back on: a "case manager salary" quoted to the dollar by a job board or aggregator is a self-reported sample of blurred titles, not a measurement of any occupation.
This page does not estimate the gap the release leaves.
A plausible band is not evidence.
Set the three tables in the field's wider ladder and the structure is legible.
The all-occupations median is $50,980, so the proxy median sits $5,050 below it while both social-work medians sit above.
Just under the proxy sit the entry tables β psychiatric technicians at $45,130 and psychiatric aides at $44,910, within a $1,020 spread of the human-service-assistant median β the floor of behavioral health pay, covered on the behavioral health technician salary page.
The step up runs through the credential: the combined counselor occupation reports $59,350, on the substance abuse counselor salary page, and the licensed clinical tier above that blends its own datasets.
The case manager's proxy table is one rung above the floor, and every rung above it is credential-gated.
State-level figures exist in the same release β BLS publishes a 21-1093 median for all 51 jurisdictions β and per-state pages for this role will be added here as they are written, from that release and no other.
Until then this page carries no state table and no "highest-paying states" ranking: with the title itself absent from the series, a state list built now would rank a proxy occupation and label it the answer to this page's question.

This page is career information, not legal, clinical, or medical advice, and it is not guidance for anyone seeking treatment. Wage figures are BLS OEWS May 2025; qualification rules for funded case management are set by state Medicaid agencies and program policy, and certification requirements by each certifying body β all of it changes, so confirm the current position with the body that makes the rule before relying on any of it.
