Iowa licenses mental health counselors on two rungs: the temporary licensed mental health counselor β a license valid three years β and the licensed mental health counselor (LMHC).
The route is a master's of at least 60 semester hours through a CACREP program or a CCE content-equivalency evaluation, at least 3,000 hours of supervised practice with 1,500 hours of direct client contact and 110 hours of supervision over at least two years, and the NCMHCE.
The quirk: a 2024 law bars the board from requiring any in-person supervised clinical experience.

At a glance
Licensed mental health counselor (LMHC), granted under Iowa Code 154D.2 once the degree, the supervised experience and the exam are met; there is no supervisor designation or clinical specialty tier above it.
Temporary licensed mental health counselor β a LICENSE, valid three years and renewable at the board's discretion, issued only for fulfilling the postgraduate supervised clinical experience requirement (Iowa Code 154D.7; 481 IAC 880.2(1)).
A master's of at least 60 semester hours (45 if you entered a program before July 1, 2012) in mental health counseling: a CACREP-accredited program, or a content-equivalent degree with a CCE equivalency evaluation.
A doctoral degree in counseling with emphasis in mental health counseling also qualifies (Iowa Code 154D.2(1)(a); 481 IAC 880.4(2)).
At least 3,000 hours of total practice with at least 1,500 hours of direct client contact and at least 110 hours of direct supervision (maximum 50 group), over a minimum of two years, begun only after all graduate coursework except the thesis (481 IAC 880.7(1)).
The NCMHCE for the full license; the temporary license accepts the NCE or the NCMHCE, and a passed NCMHCE does not need to be taken a second time.
No jurisprudence exam (481 IAC 880.3(1)).
No supervisor credential.
A supervisor needs an active Iowa license as an independent-level social worker, mental health counselor or marital and family therapist, at least three years of independent practice, and at least a six-hour CE course or one graduate-level course in supervision (481 IAC 880.7(2)).
Enacted: Iowa Code ch. 147H (2023 Acts, ch 156).
NOT live as of September 2026 β Iowa is not among the nine states issuing privileges, and no go-live date was found in the sources read.
Biennial renewal, October 1 of an even-numbered year to September 30 of the next even-numbered year; 40 hours of continuing education in a 25-month compliance period, September 1 of an even-numbered year to September 30 of the next, of which 3 hours must be in ethics (481 IAC 880.10(1); 881.2(1), 881.2(10)).
A license, not a registration or a permit: Iowa Code 154D.1(13) defines it as a person licensed to practice mental health counseling under supervision in accordance with section 154D.7, and the rule says the temporary license is only valid for fulfilling the postgraduate supervised clinical experience requirement, is valid for three years, and may be renewed at the discretion of the board (481 IAC 880.2(1)).
The full license, granted under Iowa Code 154D.2 once the degree, the supervised clinical experience and the board-approved examination are met; the rule creates no clinical specialty tier and no supervisor designation above it.
The entry rung is a license, not a registration or a permit: Iowa Code 154D.1(13) defines the temporary licensed mental health counselor as a person licensed to practice mental health counseling under supervision in accordance with section 154D.7.
The rule is blunter about its purpose: a temporary license is only valid for fulfilling the postgraduate supervised clinical experience requirement, is valid for three years, and may be renewed at the discretion of the board.
A temporary applicant files a supervision plan before licensure and remains under supervision until the permanent license is issued (880.2(1)(f)).
Where you can work is written down too: 880.20 confines a temporary licensee to an agency or group practice with oversight and at least one independently licensed mental health provider, and bars solo practice and practices staffed solely by temporary licensees.
Above it sits the LMHC, with no clinical specialty tier and no supervisor designation in the rule.
Other states issue the same counselor licence under different letters β see LPC vs LMHC vs LPCC vs LCPC, mapped state by state.
ββTemporary licensed mental health counselorβ means a person licensed to practice mental health counseling under supervision in accordance with section 154D.7.β
The statute sets the floor: a master's degree in mental health counseling consisting of at least sixty semester hours, or its equivalent, from a nationally accredited institution or a program approved by the board.
The rule names two routes.
Route one is a master's β or a doctoral degree in counseling with emphasis in mental health counseling β from a mental health counseling program accredited by CACREP.
Route two is a content-equivalent degree: graduates of non-CACREP accredited mental health counseling programs provide an equivalency evaluation of their educational credentials by the CCE.
Applicants who entered a program of study before July 1, 2012 present a 45-semester-hour master's instead.
Coursework must include three semester credit hours in each listed content area, and no course may be used to fulfill more than one content area (880.4(2)).
For why boards write program accreditation into the degree rule, see CSWE vs CACREP vs COAMFTE accreditation.
Timing is the first gate: the supervised clinical experience cannot begin until after all graduate coursework has been completed, thesis excepted, so practicum and internship hours from school do not start the clock.
Then the rule sets four minimums β a minimum of two years' duration, at least 3,000 hours of total practice, at least 1,500 hours of direct client contact, and at least 110 hours of direct supervision equitably distributed throughout, of which a maximum of 50 may be group supervision.
After the 110 hours are complete, ongoing direct supervision continues for the remainder of the experience.
The content is prescribed as well: psychosocial assessments, diagnostic practice using the DSM-5-TR published March 2022, and treatment with goals, evidence-based modalities and differential treatment planning, plus training in practice management, ethics, documentation, coordination of care and self-care.
A grandfather clause spares supervision that started before July 20, 2022 (880.7(6)).
For what counts as a supervised hour, and what changes at independent licensure, see associate vs. independently licensed therapists.
βThe supervised clinical experience will consist of at least 110 hours of direct supervision equitably distributed throughout the supervised clinical experience.β
The full license tests on the National Clinical Mental Health Counselor Examination (NCMHCE) of the NBCC, and you register for the exam directly with the NBCC.
The temporary license accepts either the NCMHCE or the National Counselor Examination (NCE).
A passed NCMHCE carries through: it does not need to be taken a second time when applying for the full license.
Take the NCE for the temporary license instead, and the NCMHCE still stands between you and the full license.
The passing score is the passing point criterion established by the national testing authority at the time the test was administered.
Iowa requires no jurisprudence exam.
For how the counselor, social work and MFT licensing exams compare, see ASWB vs NCE vs NCMHCE vs the MFT national exam.
Iowa's headline rule is statutory: the board cannot require any in-person supervised clinical experience, or live or recorded direct observation of client interaction, and the board's own rule adds that direct supervision can occur in person or by using videoconferencing (880.7(1)(e)).
Choosing a supervisor then turns on three qualifications: an active Iowa license as an independent-level social worker, mental health counselor or marital and family therapist; at least three years of independent practice; and at least a six-hour continuing education course in supervision or one graduate-level course in supervision.
There is no separate supervisor credential.
The same 3,000/1,500/110 rule governs mental health counselors, marital and family therapists and independent-level social workers alike, so a supervisor from a neighboring discipline is ordinary here.
A supervisee may use up to four supervisors at a time, must notify each about the others, and any supervisor who misses a requirement needs board approval before supervision begins.
For what it takes to become a supervisor yourself, see can an LPC supervise an associate?
βThe board shall not, by rule or other means, require any in-person supervised clinical experience or require that supervised clinical experience include live or recorded direct observation of client interaction.β
The statute's definition of mental health counseling runs from assessment, referral and consultation through human development principles, learning theory, group dynamics and the etiology of maladjustment and dysfunctional behavior.
The operative word is assessment: whether an Iowa LMHC may diagnose was not established in the sources read.
What the rule does write down is a training expectation β the supervised clinical experience must include diagnostic practice using the current edition of the DSM-5-TR, published March 2022, alongside psychosocial assessments and differential treatment planning.
If diagnosis authority matters for a position you are weighing, put the question to the board in writing before you accept it.
ββMental health counselingβ means the provision of counseling services involving assessment, referral, consultation, and the application of counseling, human development principles, learning theory, group dynamics, and the etiology of maladjustment and dysfunctional behavior to individuals, families, and groups.β
A counselor licensed in another jurisdiction applies for licensure by endorsement under Iowa Code 154D.8, a section 2024 Acts ch 1080 added.
The statute requires the applicant to show evidence that the jurisdiction in which the applicant holds a license has licensure requirements similar to the licensure requirements of this state β Iowa judges your home state's rule, not an hour-for-hour recount.
Rule 880.5 supplies the mechanics: verification of the license sent directly from the other jurisdiction, with web-based verification accepted if it shows your name, initial licensure date, current licensure status and any disciplinary action, plus disclosure of public or pending complaints.
If you cannot satisfy endorsement, 880.5(2) points to licensure by verification under rule 481β501.1(272C), if eligible.
Experience completed in another state without licensure there is a separate question β contact the board before counting it (880.7(5)).
For how endorsement works when you move, see moving states as a therapist: license transfer and endorsement.
βShow evidence that the jurisdiction in which the applicant holds a license has licensure requirements similar to the licensure requirements of this state.β
Iowa enacted the Professional Counselors Licensure Compact as Iowa Code chapter 147H (2023 Acts, ch 156, section 1).
Enactment is not operation: as of September 2026 the commission's home page does not list Iowa among the nine states issuing and accepting privileges, and no go-live date was found in the sources read.
The board's rule 880.18 already incorporates the commission's rules, lets a counselor holding a current compact privilege practice in Iowa without a separate Iowa license, and sets a $60 state fee for issuing a compact privilege to practice in Iowa, waived for active-duty military members and their spouses.
(The 880.18 text this page quotes is the IAC 9/2/26 compilation; ARC 0556D amendments to it take effect October 7, 2026.)
Until the compact is live, an Iowa LMHC cannot use it to practice elsewhere and a counselor licensed elsewhere cannot use it to practice here; the route in is an Iowa license, by examination or endorsement.
For how the compactβs privilege to practice works, see the Counseling Compact explained.
βA mental health counselor may engage in the practice of licensed mental health counseling in Iowa without a license issued by the board if the individual has a current compact privilege to practice in Iowa issued by the counseling compact.β
The license renews biennially, on a period running October 1 of an even-numbered year to September 30 of the next even-numbered year (880.10(1)).
Continuing education runs on its own clock: a 25-month compliance period beginning September 1 of an even-numbered year and ending September 30 of the next even-numbered year, with a minimum of 40 hours.
Of the 40, three hours must be in ethics, and a licensee serving in a supervisory role must complete three hours of continuing education in supervision (881.2(10)(b)).
New licensees sit out the first renewal and may use hours earned from initial licensing until the second renewal; up to 50 percent of hours carry over (881.2(4), (7)).
The statute bars the board from limiting online continuing education for mental health counselors, provided the program meets board standards (154D.3(1)(e)).
Mandatory reporter training rides on the renewal: two hours on child abuse identification and reporting within the previous three years if you counsel children, and two hours on dependent adult abuse if you counsel adults (880.10(3)).
βThe biennial license renewal period for a license to practice marital and family therapy or mental health counseling shall begin on October 1 of an even-numbered year and end on September 30 of the next even-numbered year.β
The window is busy, and all of it is paperwork you can get wrong.
2023 Acts ch 156 enacted the Counseling Compact (chapter 147H).
2024 Acts ch 1080 added the no-in-person-supervision bar in 154D.2(2) and wrote the endorsement section, 154D.8.
The rules moved twice: chapter 645β31 transferred to 481βchapter 891 (IAC Supplement 9/18/24), and 481β891 was then rescinded and replaced by the current 481 IAC chapter 880 under the merged Board of Behavioral Health Professionals, effective August 13, 2025 (ARC 9401C, IAB 7/9/25) β a guide citing 645 IAC ch. 31 or 481 IAC ch. 891 is quoting rescinded text.
One more change is already on the books: ARC 0556D (IAB 9/2/26) amends rules 880.1, 880.2, 880.4, 880.10, 880.14, 880.18 and 880.20, effective October 7, 2026.
The compiled chapter this page quotes is that post-amendment text.
This page summarizes the Iowa statute, rules, board pages and other documents cited above, as read for this state-by-state counselor licensure series, current as of September 2026.
It does not list application forms, fees beyond those cited above, or processing times, and it is not a substitute for the instructions the Board of Behavioral Health Professionals (Iowa Dept. of Inspections, Appeals, and Licensing) publishes.
Nor does it cover social work licensure in Iowa, MFT licensure in Iowa, or what therapists earn in Iowa.
Hour totals, exams and supervision rules change by statute and by board rule; for how the counselor, social work and MFT licences differ, see LCSW vs. LPC vs. LMFT: which therapy licence to get.
Confirm every requirement with the Board of Behavioral Health Professionals (Iowa Dept. of Inspections, Appeals, and Licensing) before you count hours, register for an exam or apply β and before you take one of the mental health counselor jobs in Iowa that asks for it.
No. Rule 481 IAC 880.7(1)(a) says the supervised clinical experience cannot begin until after all graduate coursework has been completed, with the exception of the thesis.
Practicum and internship hours earned while coursework is still under way do not start the clock.
If you are down to a thesis only, ask the Board of Behavioral Health Professionals how it wants that status documented before your supervisor signs an attestation of supervision.
Yes.
Iowa Code 154D.2(2) says the board shall not, by rule or other means, require any in-person supervised clinical experience or require that supervised clinical experience include live or recorded direct observation of client interaction.
Rule 880.7(1)(e) adds that direct supervision can occur in person or by using videoconferencing.
Your supervisor must still meet 880.7(2), and the 110 direct supervision hours must still be equitably distributed across the experience.
Rule 880.7(2) requires a supervisor to hold an active Iowa license as an independent-level social worker, mental health counselor or marital and family therapist, to have a minimum of three years of independent practice, and to have completed at least a six-hour continuing education course in supervision or one graduate-level course in supervision.
There is no separate supervisor credential.
You may use up to four supervisors at a time, and you are responsible for notifying each one about the others.
Three years, renewable at the discretion of the board (Iowa Code 154D.7; 481 IAC 880.2(1)).
The license is only valid for fulfilling the postgraduate supervised clinical experience requirement, and rule 880.20 says a temporary licensee may practice only as part of an agency or group practice with oversight and at least one independently licensed mental health provider β not as a solo practitioner and not solely with other temporary licensees.
You remain under supervision until the permanent license is issued.
Not as of September 2026.
Iowa enacted the Professional Counselors Licensure Compact as Iowa Code chapter 147H in 2023, but the Counseling Compact Commission's home page does not list Iowa among the nine states issuing and accepting privileges, and no go-live date was found in the sources read.
Until then, the route to practicing in Iowa is an Iowa license, by examination or by endorsement.
Rule 880.18 sets a $60 state fee for a compact privilege once one is available.
